4.1 says:

  # In this specification, the expressions "immediately before" or
  # "immediately after" mean with no intervening white space or
  # comments.

However, the spec consistently uses "immediately followed" or "followed
immediately" (or "follows" etc) in this context, so we should consider making the following change:

  | comments.
