I think the solution to this is going to have to be a fragment identifier syntax that can directly clip from the image. This would be useful in many other contexts also.

However, that's out of scope of HTML5, so I'm marking it WONTFIX for here. I encourage you to take this up on one of the relevant lists; maybe uri@w3.org might be able to point you in the right direction.
