An erratum (E1-56) resolving this issue was approved (with modifications) by the XML Schema WG at its meeting of 9 July 2010.  The modified erratum was published in the 1.0 Second Edition errata document at

Just for the record:  two bugs against XDM (and possibly more that I haven't found) seem to be tied up with this topic:  Bug 2630 [DM] Default values of elements and Bug 5319 [DM] Default value now well-defined for mixed content.  We hope the resolution of this bug and the issuance of the erratum will be helpful to QT in finally laying those bugs to rest.
