In the most recent internal draft from Aug 17, 2015, four reference errors can be found, signified by the appearance of "???".

Under:
* 18.3 copy-of (link to accumulators)
* 20.3.4 fn:system-property (penultimate bullet, link to an extensions section)
* 21.2.4 map:keys (link to non-deterministic with respect to ordering)
* 21.2.10 map:for-each (link to non-deterministic with respect to ordering)

These have been fixed (silently).

(The references in map:keys and map:for-each are awkward to maintain because the link markup is different in the XSLT spec and the F+O spec).
