This test uses the environment

   <environment name="accessor004">
      <source role=".">
         <content><![CDATA[ 
<doc/>]]></content>
      </source>
   </environment>

and expects a non-empty value for document-uri.

Should a URI not be specified on this <source>?

I've made accessor-004 into a "real" file (no longer inline) as that seems to make more sense when there are tests using base-uri(), doc(), and document-uri().
