The spec is inconsistent on this point.

I was asked to open this issue for better tracking. There has already been substantial discussion on the (member-only) w3c-xsl-query@w3.org mailing list. The thread starts here:

but I think the best place to start is with this post:

because it contains a statement of the problem, an overview of possible solutions, and a specific proposal.
