We have actually added some new text to the CCXM spec around error  
handing. Here is a snippet of it:

Our feeling is this should resolve your questions around the error  
handing. Please let us know if this resolves the issues for you by Feb  
22nd 2008. If we do not hear from you by that date we will consider  
the issues closed.
