Related notes:
DOM Parsing and Serialization includes the method insertAdjacentHTML which allows elements/text to be injected in the DOM after the page is loaded. I don't know if elements/text injected with this method gets to the accessibility tree. May be related to CSS injected content not getting into the accessibility tree. I suggest Joanie or a browser vendor be asked about whether this method injects content into the DOM that does not get in the accessibility tree.
Updated Spec review wiki page with these comments. https://www.w3.org/WAI/APA/wiki/DOM_Parsing_and_Serialization
Leonie and Cynthia said stuff put in the DOM using the method does get into the accessibility tree.
