Add a "Not Applicable" button to the data entry form.

Added 'na' to possible result in the results table.

Checked in to CVS entire updated database structure used by my prototype (css2test.sql).

This fix allows a "not applicable" button to be added in the future.

Actually adding the button is differed, pending an additional request.
