Update on my action on bias review

Hello everyone,

As per my action item from last week [1] to review the bp document and
check whether my previous suggestions were covered. I have updated the
initial comments document [2] with additional comments to indicate those
that have been "Solved", those that were just "Removed" from the new
document and those that still "Remain"
