Full R2RML Review after ISSUE-68 and ISSUE-73 are resolved

I intend to do another full review of the R2RML draft as it has been
updated to reflect last call comments. However, I plan to wait to do this
until ISSUE-68 [1] and ISSUE-73 [2] are addressed because these issues will
affect details of the text of the spec in many places.
