Well, there are still lots of bugs, I'm sure, and various issues with
the Presentation Syntax(es), but the basic thing works.  Or, at least,
it can handle the XML in BLD example 7.

(I did all four parts - read & write PS, read & write XML.  The
XML-writer does several things wrong currently.)
