I have restored the qualifiers as they appeared in the most recent document that had them, and tried to integrate them into the document, the text, and the BNF.

PLEASE NOTE that the list of qualifiers is expected, I think, at some point, to match the list of permissions in the compliance document.  It does NOT DO SO right now, simply because I thought (a0 I should restore what was there first and (b) the permissions seem in flux right now.
