missing request and response headers

While patching the ldp-primer and cross referencing points in the LDP spec, 
I noticed a major improvement that could be made in the LDP spec as far
as the examples go: each example should be preceded with the request made to it,
and also the responses should contain the headers. 

This would make it much more likely that someone reading the spec would 
understand what is required.
