Examples of hierarchical chapters from DAISY can be found here: http://www.daisy.org/sample-content - take in particular those that say "demonstrating DAISY navigation".

Hmm, from the description of this bug it looks like the same suggestion, basically. This bug was created by Hixie, so I guess I'll leave it to him to understand what he means :)
