it is unclear what the presentation semantics of control and other unmapped characters should be with respect to background color, default glyph, etc.; need to check if this is handled (indirectly) by XSL-FO semantics or not, and if not, then add some guidance and possibly normative text

I believe this is indeed handled by XSL:FO. Essentially only the glyph areas created for characters designated as white space by XML (ie (#x20 | #x9 | #xD | #xA) get deleted when building line and glyph (see 4.7.2 Line-building part 6). Otherwise the glyph is taken from the nominal-font (which could be a system font) and will display the 'missing char' representation from that font if no explicit glyph is provided. (see section 5.5.7 Font Properties, Section 7.9.3 "font-selection-strategy")
