Seems a majority is in favour of adding titles / lang by default to test cases

scribe: not sure how to keep the test case IDs from staying unchanged

conclusion: agreed to make the change. We'll take doctype out of the hash, this will change all hashes so far. We'll add a template that will be used when there is no doctype and no HTML element.
... we'll make these changes when things have stabalized a little more, as to not impact exiting work

Conclusion: the group things disabled form controls still need a label. Having an adjecent text is insufficient for all cases where disabled form controls can be encountered
