Example case:
* document without declared encoding at any level, encoded in e.g latin1
* validator preparses, finds no encoding, throws warning, defaults to utf8
* decoding errors, validator throws fatal error
* output shows only fatal error.

The warning about lack of encoding should be shown, too.
Need to fix template order/logic.

Targeting upcoming 0.7.3

Fixed in current 0.7 code (ready for 0.7.3 release as targeted) and in the next development version as well.
