The text for example in 3.3.3, Combining Node Sequences, should state that the nodes with symbolic names A, B, C are in document order (i.e. A << B << C ).

If they weren't, the given operation results would be incorrect, since a combining operation returns the sequence of nodes in document order.

I agree. I've made this change in our internal Working Draft.
