"For each [Attr] in the [Node.attributes] list that has a [namespace prefix] value of xmlns, create a [URI mapping] by storing the [local name] as the value to be mapped, and the [Node.nodeValue] as the value to map."

This should actually look at the namespace *prefix* not the namespace URI.

I don't follow - isn't that what the current text says? Where does the current text talk about "namespace URI"?
