While checking the coverage and error coverage of the Full Text Test Suite, I observed that there are not yet any tests for three errors: FOCH0002, XQST0013, and XQST0079, even though all three of those errors are used in the text of the spec. 

Descriptions should be added to Appendix D, Error Conditions, for those three error codes.

Apologies! I intended to say that I further discovered that Appendix D does not contain descriptions of three error codes: XPST0003, XQST0013, and XQST0079.  Descriptions should be added to Appendix D, Error Conditions, for *these* three error codes.

Changes have been made that add entries for and descriptions of each of the three errors that were identified as missing from Appendix D: XPST0003, XQST0013, and XQST0079.  We have thus marked this bug FIXED.  If you agree with the changes, please mark it CLOSED.
