The terms 'definition' and 'declaration' seem to be used more or less
interchangeably in the current version of the spec.  It would be good to
decide on a consistent usage and stick to it.

Changed usage of 'declaration' and 'definition' as pertains to elements, attributes, and types in accordance with XML Schema 1.1 Spec.
