Title: Section 2.9 should also explain basic attaching (inline)

Description: In section 2.9 explain/show the three different ways to attach a policy in WSDL: inline, reference and external reference. 

Justification: Someone new to WS-Policy should be introduced to all the fundamental uses of policies and how the are assigned/attached to WSDL- not just references. The Primer should start basic and build up the descriptions and motivations for other usage patterns (references etc.)

Target. Primer Section 2.9 and perhaps the advanced section (?)

Proposal add the following text to section 2.10.  I suggest putting it as paragraph 2 of the section. (We should discuss the proper placement) :

Section <a href="#simple-messagel"><b>2.2 Simple Message</b></a>, showed how a policy expression can be attached directly to a binding inline. A single policy expression may be used in several parts of a WSDL document.  In this case it is desirable to use references to the policy expression rather than to directly inline the policy expression.
