Call out that lowercase/uppercase is only affected query values. While case sensitive/insensitive affects comparison. Also call out which ones can be set in static prolog and which ones can be set on query (Appendix C only allows case/diacritics * and not the with/without for obvious reasons).

This bug is in part a duplicate of Bug# 3924.  The Task Force is closing this bug now, and will close #3924 when the changes have been made.
