Should the two operations (insert and insert as last) be commutative or not, i.e. should they be marked as conflicts or should we define a proper semantics for such a combination ?

"Insert" and "Insert as Last" are compatible operations. "Insert" has an implementation-dependent position that must not conflict with "Insert as Last". This resolution has been documented in a working draft.
