The last sentence in section 5 (XML Output Method) should be moved to section 
5.1.10 (XML Output Method: the use-character-maps Parameter).  The description 
of error err:SERE0003 provided in the beginning of section 5 already points 
out the same information though worded differently.

"The result of serialization using the XML output method is not guaranteed to 
be well-formed XML if character maps have been specified (see 9 Character 
Maps)."
