Rename "first initialization segment flag" to "first initialization segment received flag" to make the spec text less confusing. This flag is set to true once the first initialization segment has been received and processed by the Initialization Segment Received algorithm. 

The current name implies that the flag indicates we are handling the first initialization segment which is not true. This also makes UA code that tries to use the same names as the spec confusing.
