In the "when entering a heading content element" section, there's wording like
"rank equal to or greater", but ranks are instead referred to as "higher" or
"lower". It's not clear whether "greater" means "higher" or "bigger number"
(lower). This section should consistently use "equal to or higher/lower"
instead.

This bug was cloned to create bug 18099 as part of operation convergence.

I found one case of "greater". It was a mistake. I've changed it to "higher", which I'm pretty sure is what was intended. Let me know if anything is ambiguous still.
