2.1.4 Notations for environments
Regarding environment groups, you list what you call the "two main
environment groups" namely the dynamic context and the static context.
It's nice to have examples, but it's even better to have complete
lists.  Please list all environments, or supply a pointer to
somewhere that does.

Changed to state that there are exactly 2 environments
in which all others are defined.
