2.2 URIs, Namespaces, and Prefixes

"None of these special prefixes are given a URI."
    That's going to cause problems when you try to conclude (e.g.)
        fs:dot of var expands to expanded-QName
    or
        op:numeric-add of func expands to expanded-QName

Added an indication that they behave as having a corresponding URI for the
purposes of namespace resolution.
