The SOAP 1.1 fault example in Section 6 does not have the required wsa:Action header. This may confuse people to think that the header is optional.

Proposal:
copy the header from the SOAP 1.2 example above it.

Obviously this applies to all of our specs since they all share the same
template.  But what's funny is that I just took a look at some other
WS* specs (RM and Tx) and they both do the same thing. No one ever noticed
this before.

Of course, the WSA spec itself doesn't have this mistake  :-)
