There are cases where frag says things like:
  When this Mode value is used the Value element MUST be present.
or
  When this Mode value is used the Value element MUST NOT be present. 

what it doesn't say what to do if this condition is violated.

Proposal:
add to the end of each:
... be present, and if it then a wsf:InvalidInput fault MUST 
be generated.
