If I remember correctly we decided to remove manually populated indexes, at least for the time being. We need to scan the spec and remove all references to manually populated indexes (and probably remove references to "automatic" population and just assume that's the case).

That is correct.  Any such remaining references were likely just an editorial oversight.

Are there any specific instances of this that you're worried about?  If not, I'm not sure this bug is helpful.

Just quickly scanned the spec and didn't find any more references to manually maintained indexes, so I'm resolving this bug. As we do further edits let's keep an eye on less obvious references to those and adjust things as necessary.
