When is XQST0045 raised? 

If an implementation decides to ignore the SHOULD, it presumably does not raise the error, and the text should be rewritten to clarify this.

If a user decides to ignore the SHOULD, is it an error or not? Or is it implementation-defined or implementation-dependent?  This is currently fuzzy.</thetext>
