I found that steps 6.4 and 6.5 seem to be out of order: 6.4 Otherwise, if last node is the furthest block, then move the aforementioned bookmark to be immediately after the node in the list of active formatting elements. 6.5 Create an element for the token for which the element node was created, replace the entry for node in the list of active formatting elements with an entry for the new element, replace the entry for node in the stack of open elements with an entry for the new element, and let node be the new element. Notice how 6.4 makes a bookmark relative to "node" and then 6.5 changes what "node" is. The naive implementation would cause the bookmark's anchor node to fail to be found.
