Add ability to define a profile from multiple profiles.

Description:
It should be possible to define a profile that consists of multiple nested profiles, i.e., whose children are all ttp:profile instead of ttp:{feature,extension}. The new ttp:profileCombination would apply when combining the nested profiles into the resulting combined profile definition.

EBU liaison has requested feature to allow document conformance to (zero to many) profiles to be described using short form aliases to allow references to specifications not wholly defined by schemas or feature designators, i.e. where behaviour is described normatively in documentation.

EBU did not request that processors must be able to perform combinatorial logic on profile features, however it is an EBU requirement that some features may be considered 'optional extensions' that, if not supported, will not result in a non-renderable document, i.e. it would lead to a non-ideal but still acceptable presentation; therefore a processor not supporting those optional extension features should continue to process the document regardless. Such optional extension features should be distinguishable from other genuinely required features.

The definition of 'optional extension' vs 'required' features may be contained within specifications identified via a short form alias instead of, or additionally to, features listed within profile statements inside each document.
