When a default/fixed attribute/element value is of type QName/NOTATION, how is 
it applied to the instance? For example: <attribute name="att" type="QName" 
xmlns:p1="my_uri" default="p1:local"/> If the above attribute doesn't appear in 
an instance, we should add a new attribute. But what would be the (string) value 
of the added attribute? (Note that the prefix "p1" might not be declared in the 
instance, or it's possible to be bound to a different namespace uri.)

The email from Jonathan Marsh to the XML Schema WG on 24 January 2004
under the subject line "Schema infoset fix-ups" is also relevant

This is the second of three issues raised in that email. The first is
recorded in bug 2102 (for 1.0) and 2105 (for 1.1); the second in bug
2103 (for 1.0) and bug 2748 (this issue, for 1.1); the third in bug
4159 (for both versions).

A proposal to resolve this issue was approved at the WG's meeting in
New Orleans at the end of January.  Final amendments to the wording 
were approved in the call today, and the wording is being integrated
into the status quo text.

Accordingly, I'm marking this issue as resolved.  Because the original
request has NOT been fulfilled, I'm marking the resolution as
WONTFIX.

The short description of the resolution is:  the infoset fixup described 
in the issue description is implementation-dependent.

The originator of the issue, Jonathan Marsh, has been contacted separately,
as will the XML Core Working Group, which is responsible for the
XInclude spec from which the issue arose.  See
