For the most part, our TTS engines figure out what language text is using heruistics. UIA does have a text attribute UIA_CultureAttributeId which takes the standard culture strings (en-us etc). Word exposes this. I'm not sure if Narrator consumes it for Word.
Edge and IE do not expose this, and rely on TTS engines to figure it out.
