How do we want to specify rendings. There are a couple of options:

* as an image: universal, but not helpful for implementors
* as presentation MathML
* multi-format

in the latter cases: what do we do for role:application? There are solutions
from ORCCA or OMDoc

JSD: Isn\'t this an issue for the spec as a whole, and not just content mathml.

I think we should provide presentation markup, and then deal with all the
presentations in the spec in a unified manner.
