Description:
Can an empty start request take its content from a previous startrequest, or only from a preparerequest. Do we need emtpy startrequest at all? If we keep empty startrequest, then at the very least the MC must be required to keep track of the markup in a preparerequest.

Decisions from minutes:
Jim: the PrepareRequests are in a queue. An empty StartRequest will use the latest PrepareRequest. A non-empty request will use whichever one it references.
... if the list is empty, or if your start request references a special URL, you get the default behavior of the MC.

Decision is if startRequest arrives while the MC is in Runnig or in Paused, re-start the MC. Use content following ordinary rules for StartRequest.
