How to Retrieve an Annotation
The fields of the form are ('*' denotes the field is required):
Username* the username for the specified service
Password* the password for the specified service
Document* - the URI of the document to retrieve
Annotation service* - the address of the Annotea service to query
Servlet name* - the address of the Annotea servlet
